Grant Johnston

SVP, Asset Management

Mr. Johnston joined NHI in July 2025 as Senior Vice President of Asset Management and brings over 20 years of healthcare finance, real estate and asset management experience to the Company. Mr. Johnston oversees NHI’s asset management group and is responsible for fostering strategic partnerships, managing complex transactions, and guiding investment decisions that support the company’s real estate investment portfolio.

Most recently, Grant led asset management and development at Synergy Healthcare Solutions (Synergy) where he implemented restructuring initiatives, underwrote real estate and operating investments, and managed lending and landlord relationships for affiliated and 3rd-party skilled nursing investors and providers. Prior to joining Synergy in 2020, he served as SVP in Formation Capital’s (Formation) asset management group focusing on debt refinancing, restructuring and new investments for Formation’s portfolio of healthcare real estate and operating company investments. His career in healthcare began as an associate in GE Capital’s Risk Management Leadership Program (2003-2005) progressing to VP of Underwriting and Portfolio Management at GE Capital, Healthcare Financial Services specializing in asset based and cash flow transactions for healthcare companies.

Grant is a CPA, holds an International Master of Business Administration from the University of South Carolina and earned a Master of Accounting, Bachelor of Business Management and a Bachelor of Spanish from North Carolina State University.
